The Public Health Committee at the Willow Clinic is focused on expanding access to community resources and supporting patients beyond the clinic setting. We work to connect patients with social services, provide education on available resources, and strengthen partnerships that improve well-being.
Resource Navigation & Referrals
We identify and compile community resources to support patients with needs such as food access, housing, mental health services, and substance use support. Our goal is to make these resources easily accessible through pamphlets and referrals.
Community Partnerships & Outreach
Our committee builds relationships with local organizations to expand the network of support for our patients. We also conduct site visits and collaborate with partners to better understand and share available services.
Education & Awareness
We create informational materials and present resources to our patients to educate on available services and how to access them.
